If your clinic appears on LongevityMap without you having claimed it and you prefer not to be listed, complete this form. We will review your request within a maximum of 30 days.

Minimum 20 characters. We may ask for additional documentary proof before removing.

Your request is registered with a time stamp (GDPR Art. 30). We will manually verify ownership before removing.
