Draining the aging brain: restoring glymphatic clearance halts neurological decline

Pharmacological activation of aquaporin-4 (AQP4) via the compound TGN-073 restores cerebrospinal fluid drainage and reverses markers of neurological aging in tau-transgenic mice. The drug enhances water flow across cell membranes, potentiating the glymphatic system—the brain's waste-clearance network that progressively fails with age. In PS19 mice (a tauopathy model), chronic TGN-073 treatment significantly reduced tau accumulation, neuronal loss, and gliosis while increasing tau levels in cerebrospinal fluid, indicating enhanced clearance. Critically, these benefits disappeared entirely in AQP4-deficient mice, establishing target specificity: the improvement depends on AQP4, not off-target effects.
