Gut-derived TMAO accelerates atrial fibrillation: a new therapeutic target

TMAO, a molecule generated when the gut microbiota metabolizes dietary choline, emerges as an independent risk factor for idiopathic atrial fibrillation, the most common form of age-related cardiac arrhythmia. In a cohort of 5,090 cardiac catheterization patients, elevated TMAO levels independently associated with prevalent atrial fibrillation at an adjusted odds ratio of 1.7. Researchers identified the mechanism: TMAO inhibits muscarinic receptor 2, triggering autonomic dysfunction that precipitates arrhythmias. In transgenic mouse models of atrial fibrillation, a TMAO-enriched diet accelerated disease onset, while choline supplementation exacerbated these effects—findings reversed by iodomethylcholine, a compound that blocks the microbial enzyme converting choline to TMAO. This pharmaceutical intervention reduced circulating TMAO levels and suppressed choline-induced arrhythmia induction, offering a non-invasive alternative to cardiac ablation for idiopathic cases.
