Definition and how it works: Ozone therapy is a medical treatment that uses a therapeutic mixture of ozone (O₃) and oxygen at controlled concentrations. It is administered via several routes: intravenous (major autohemotherapy), intramuscular, rectal or local. Ozone is a potent oxidant that, at therapeutic doses, activates the endogenous antioxidant system, improves tissue oxygenation and exerts antimicrobial, immunomodulatory and anti-inflammatory effects.

Origin and history
Ozone was first used for medical purposes during the First World War to disinfect wounds. Its systemic use was developed in Germany by Dr Hansler in the 1950s–60s and spread globally from the 1980s onwards.
1. Pre-treatment analysis Mandatory G6PD test. Hepatic and renal function blood panel required prior to treatment.
2. Preparation of the mixture Certified O₃/O₂ mixture prepared using a calibrated generator. Concentration: 20–40 μg/ml.
3. Major autohemotherapy Extraction of 100–200 ml of blood, ozonisation in a sterile glass flask and slow intravenous reinfusion.
4. Monitoring Vital signs monitored throughout reinfusion. Total session duration: 45–60 minutes.
5. Session protocol 10–20 sessions in the initial phase (2–3 per week). Maintenance: 1 session per week.

The primary mechanism is activation of the Nrf2 transcription factor, which regulates more than 200 antioxidant genes. Studies published in Free Radical Biology and Medicine confirm the induction of HO-1, SOD and GPx following ozone therapy. Major autohemotherapy (AHT) has demonstrated reductions in IL-6 and TNF-α in controlled clinical trials for rheumatoid arthritis and fibromyalgia.
SR/MA 7 RCTs (n=409 knee OA): intra-articular ozone injections more effective than corticosteroids at reducing pain in the short and medium term, with comparable functional improvement. Methodological quality limited.
Pilot RCT 2024 (n=73, post-acute COVID-19 sequelae): major ozone autohemotherapy + conventional treatment achieved 71% response vs 45% with conventional alone. Improved symptoms, pulmonary function (6MWD), inflammation and cellular immunity.
SR/MA 12 studies: ozone therapy reduces the size of traumatic and autoimmune oral ulcers vs placebo (RR -0.44) and reduces pain more than placebo, equivalent to topical corticosteroid. Quality of evidence limited.
SR/MA 6 studies (n=178 medication-related osteonecrosis of the jaw in oncology): adjunctive ozone therapy achieved 71% clinical success rate (95% CI 55-84%) with no adverse events. Very low certainty of evidence; standardised RCTs pending.

Major autohemotherapy — in which ozone is mixed with blood drawn from the patient before being reinfused — is safe when performed by trained medical personnel using certified equipment. Direct intravenous injection of ozone is contraindicated.
The indications with the strongest evidence include disc herniation, osteoarthritis, vascular ulcers, chronic infections and chronic fatigue syndrome. It is also widely used in preventive medicine and longevity protocols.
For chronic conditions, cycles of 10–20 sessions are recommended. For maintenance and longevity purposes, many patients continue with one session per week on an ongoing basis.
