What is Plasmapheresis?

Plasmapheresis is a procedure in which the patient's blood is drawn, the plasma is separated from the blood cells via centrifugation and filtration, and the cells are reinfused with fresh plasma (albumin or young donor plasma). In the longevity context, it is used to remove pro-inflammatory factors that accumulate with ageing (TGF-β1, β2-microglobulin, VCAM-1) — termed 'rejuvenation-inhibiting factors' by researcher Irina Conboy.

Origin and history

Medical plasmapheresis was developed in the 1960s to treat autoimmune diseases. Its application in longevity stems directly from Conboy's parabiosis studies and the research of startup Alkahest (acquired by Grifols) from 2014 onwards.

Plasmapheresis protocol step by step

1. Haematological assessment Coagulation, calcium, renal and cardiac function. Extended informed consent.

2. Vascular access Central or large-bore peripheral venous access. Citrate anticoagulation.

3. Plasma separation Continuous centrifugation in a cell separator. Selective plasma extraction (1–1.5 plasma volumes).

4. Albumin replacement Reinfusion of cells with 5% human albumin or physiological saline. 2–4 hours total.

5. Post-procedure monitoring Calcium, blood pressure and symptom monitoring for 1–2 hours post-procedure. Same-day discharge if uneventful.

The science behind Plasmapheresis

The Irina and Michael Conboy group (UC Berkeley) published in Nature (2005) that parabiosis (circulatory union of young and old mice) rejuvenated the old mouse's tissues. In 2022, they demonstrated that plasmapheresis (dilution of aged plasma) produced equivalent rejuvenation effects without needing young plasma. Clinical studies in Alzheimer's (AMBAR trial, Grifols) show slowed progression with periodic plasmapheresis.

Standard medical plasmapheresis removes a specific component (antibodies, LDL). Longevity plasmapheresis aims to dilute and remove pro-ageing factors from the plasma, replacing it with albumin or saline. They have different mechanisms.

Is there solid clinical evidence in healthy humans?

The strongest human evidence is the AMBAR trial in Alzheimer's. In healthy individuals for longevity, the data are promising but primarily from animal models. Some centres in Spain and the US offer experimental protocols under extended informed consent.

How long does a session last?
