The bioactive triterpenoids behind reishi's anti-inflammatory potency

Ganoderic acids are a group of highly oxidized, lanostane-type tetracyclic triterpenoids and the principal bioactive compounds of the medicinal mushroom Ganoderma lucidum (reishi or lingzhi). More than 150 distinct ganoderic acids have been identified, with ganoderic acid A (GAA) among the most studied. Unlike reishi's water-soluble β-glucans, these triterpenoids are fat-soluble and require alcohol extraction. Their documented effects include anti-inflammatory, hepatoprotective, anti-tumor and neuroprotective activity, mediated largely through modulation of the NF-κB, MAPK and TLR-4 signaling pathways.

Ganoderic acids account for reishi's characteristic bitterness and much of its pharmacological profile. Their oxygenated lanostane scaffold lets them engage multiple cellular targets. The best-characterized anti-inflammatory mechanism is downregulation of TLR-4/NF-κB and MAPK signaling, which lowers production of pro-inflammatory cytokines such as TNF-α, IL-1β and IL-6 — a central axis of the inflammaging associated with aging.

In experimental oncology, reishi triterpenoids induce apoptosis and arrest the cell cycle by inhibiting β-catenin and protein kinase C in carcinoma cell lines. Hepatoprotective, antioxidant (Nrf2 activation) and neuroprotective activity have also been described.

Bioavailability depends critically on the extraction method: β-glucans are pulled out in hot water, but fat-soluble ganoderic acids are only released with ethanol. That is why the canonical form is a dual extract (water + alcohol) standardized to 4-7% triterpenes, avoiding mycelium-on-rice products that contain almost no triterpenoids. Most of the evidence remains preclinical (in vitro and animal models); human clinical trials are still limited.
