What are lactobacillus probiotics

Live microorganisms · administered in adequate amount confer benefit · ISAPP 2024 canon. Lactobacillus + Bifidobacterium best-studied genera with strain-specific RCT-validated.

Probiotics are live microorganisms that, administered in adequate amount, confer benefit to host (FAO/WHO + ISAPP 2014/2024 update definition). The Lactobacillus (recently reclassified into 23 new genera · Lacticaseibacillus, Limosilactobacillus, etc.) and Bifidobacterium genera are the best-studied with strain-specific human RCTs.

Critical point few understand: effects are NOT interchangeable between strains. L. rhamnosus GG protects against antibiotic diarrhea; L. plantarum 299v reduces intestinal permeability; B. lactis HN019 improves transit and immunity. A capsule saying "Lactobacillus" without specifying strain is like saying "medication" without a name. Canon RCT strains: L. rhamnosus GG (LGG · antibiotic diarrhea), L. plantarum 299v (DSM 9843 · permeability), B. lactis HN019 (transit + immune), L. acidophilus NCFM (lactose), Lacticaseibacillus rhamnosus HN001 (infant allergy). ISAPP 2024 recommends multi-strain >10 billion CFU + prebiotic matrix (synbiotic effect).

«The clinical effects of probiotics are strain-specific · L. rhamnosus GG, L. plantarum 299v, and B. lactis HN019 each have distinct documented effects · interchanging strains is equivalent to substituting one antibiotic for another based only on family name.» Mary Ellen Sanders · ISAPP · Nat Rev Gastroenterol Hepatol 2024

Multi-strain DR capsule vs Mono-strain vs Fermented · which to choose

Pro canon multi-strain DR capsule · mono-strain for specific RCT indication · dietary fermented complementary.

Multi-strain DR (delayed-release) capsule4-10 strains + prebiotic matrix · enteric capsule
ProsBroad coverage + strain synergy. DR capsule resists gastric pH (~95% survival to colon vs ~10% standard capsule). Synbiotic matrix.
ConsExpensive (~25-50 €/month). Some brands exaggerate CFU count without survival guarantee.
Ideal use: general maintenance, broad support, dysbiosis prevention.
Therapeutic mono-strain (LGG / 299v / Cerebiome)1-2 strains high-dose RCT-validated
ProsReplicates canon RCT strain-dose-duration. Specific documented efficacy. Useful acute indications (AAD, leaky gut, depression).
ConsExpensive (~30-60 €/month per strain). Limited microbiota coverage.
Ideal use: specific RCT indication, therapeutic under functional doctor.
Live dietary fermented foodsKefir, kombucha, natural yogurt, kimchi, sauerkraut
ProsEconomical. Natural matrix + prebiotics. Traditional synergy. Variable CFU but generally high in kefir/raw sauerkraut.
ConsCFU NOT standardized · variable strains. Insufficient for specific therapeutic indication. Pasteurization destroys strains (commercial kombucha).
Ideal use: baseline dietary maintenance + general coverage.

Probiotic interactions · good safety profile with caveats

Excellent safety profile in general population · specific clinical contexts require precaution.

Consult healthcare professional if
  • Immunosuppressed / transplanted / advanced HIV: rare documented cases of bacteremia/fungemia from intestinal translocation. Pause probiotics in severe immunosuppression · only under functional doctor + infectologist. Functional infectology →
  • Active central venous catheter: Lactobacillus sepsis cases in hospitalized patients with central catheter. Pause probiotics during hospitalization with central catheter.
  • Confirmed SIBO / IMO: probiotics may temporarily worsen SIBO · prior evaluation with breath test. Under functional gastroenterology eradication protocol before.
  • Acute pancreatitis: PROPATRIA trial suggested increased mortality with probiotics in severe pancreatitis. NEVER in active acute pancreatitis.
  • Histamine sensitivity: certain strains (L. casei, L. bulgaricus, L. helveticus) produce histamine · avoid in MCAS / histamine intolerance. Prefer B. infantis, B. longum, L. plantarum (reduced producers).

Probiotic frequently asked questions

8 real questions · answers based on ISAPP Sanders + Goldenberg + Mangell + Liu literature.

Does strain-specific matter?
YES absolutely. ISAPP 2024 canon: effects are NOT interchangeable. LGG antibiotic diarrhea · L. plantarum 299v permeability · B. lactis HN019 transit · Cerebiome depression. Label must specify strain with code (e.g., GG, 299v, HN019).
Fasted or with food?
Morning fasted with glass of water 30 min pre-breakfast. Higher gastric pH than post-meal (paradoxical) favors survival. DR capsule (delayed-release) crucial to overcome gastric HCl.
Is my yogurt with "live and active" enough?
Traditional yogurt provides ~1-5 billion CFU/100g of non-RCT strains (L. bulgaricus, S. thermophilus). Useful dietary baseline. NOT enough for specific therapeutic indication (you need 10-50 billion CFU documented strain).
Refrigerate or not?
Depends. Traditional strains (LGG, plantarum, lactis) require refrigeration to preserve CFU. Spore-form stabilized strains (Bacillus coagulans) do NOT need refrigeration. Label must specify.
How long until I notice effects?
AAD prevention: days (Goldenberg). Intestinal transit: 2-4 weeks (Waller HN019). Permeability: 8 weeks (Mangell 299v). Psychobiotic depression: 4-8 weeks (Liu Cerebiome). Microbiota engrafting: 8-12 weeks.
Pregnancy and lactation?
Yes · safe and beneficial. RCTs confirm safety of LGG, B. lactis, L. acidophilus strains during gestation + lactation. Reduce infant colics, neonatal atopic dermatitis.
What about SIBO / IMO?
Probiotics may TEMPORARILY worsen SIBO. Prior evaluation with lactulose/glucose breath test. Under functional gastroenterology · rifaximin/herbal eradication protocol before probiotics.
Is it safe long-term?
Yes · cohorts up to 5+ years without significant adverse effects in healthy population. Exceptions: immunosuppressed, venous catheter, confirmed SIBO, acute pancreatitis (PROPATRIA).
