Definition
High-sensitivity CRP (hsCRP, high-sensitivity C-Reactive Protein) is an acute-phase protein synthesised by the liver in response to systemic inflammation. The high-sensitivity assay detects levels from 0.1 mg/L, compared to standard CRP (lower limit of 5 mg/L), which enables identification of chronic low-grade inflammatory states. hsCRP levels >3 mg/L in the absence of active infection are associated with double the risk of myocardial infarction, independently of cholesterol levels.

The JUPITER study (2008, NEJM, 17,802 patients) demonstrated that hsCRP predicts cardiovascular events independently of LDL cholesterol, and that statin therapy (rosuvastatin) in individuals with normal LDL but elevated hsCRP reduced cardiovascular events by 44%.
In longevity protocols, hsCRP is used as a response biomarker: an effective intervention should reduce chronic hsCRP. Target values: - Optimal: <0.5 mg/L - Low risk: 0.5–1.0 mg/L - Moderate risk: 1–3 mg/L - High risk: >3 mg/L
Do not measure during active infection (even mild) as this produces false positives.
Interventions that consistently reduce hsCRP: visceral fat loss, regular exercise, Mediterranean diet, omega-3 fatty acids (≥2 g/day EPA+DHA), statins, metformin, and several longevity therapies (cryotherapy, NAD+ IV, ozone therapy).
