Taken as a single 1500 mg daily dose, it achieves stable plasma glucosamine concentrations (~10 μmol/L) — the threshold needed to inhibit interleukin-1-induced expression of pro-inflammatory genes in cartilage. It is the only form that, in controlled trials, reduces joint pain to a statistically significant degree and shows disease-modifying effects in knee osteoarthritis, unlike non-crystalline sulfate salts or glucosamine hydrochloride.

Pharmacokinetic studies show that only pCGS — thanks to its crystalline stabilization with sodium chloride and its once-daily 1500 mg dosing — consistently reaches the plasma (~10 μmol/L) and synovial concentrations required to act on the chondrocyte. Generic formulations split into multiple smaller doses fail to reach that threshold.

At the molecular level, that concentration inhibits the IL-1β pathway, lowering expression of catabolic genes (matrix metalloproteinases, aggrecanases) that degrade cartilage matrix. Long-term trials showed a sustained reduction in the need for total knee replacement persisting up to 5 years after treatment stopped — an effect not replicated by other glucosamine forms.
