What is lactoferrin

80 kDa iron-binding glycoprotein · present in colostrum, breast milk, mucosa. Apo-form (no Fe) and holo-form (Fe-saturated). Bovine bLF is canon supplementation source.

Lactoferrin (LF) is an 80 kDa glycoprotein of the transferrin family that binds iron with very high affinity (Kd ≈10⁻²⁰ M, 260× transferrin). Present in colostrum (~7 g/L first milk), mature breast milk (~1-2 g/L), tears, saliva, intestinal/respiratory mucosa, secondary neutrophils.

Used supplement: bovine lactoferrin (bLF) · 70% sequence homology with human hLF · clinically interchangeable. Two forms: apo-LF (iron-unsaturated, <5% Fe) and holo-LF (Fe-saturated). Apo-LF preferred for anemia (leaves free Fe-binding capacity in gut) and antimicrobial (deprives pathogens of Fe). Mechanisms: anemia repletion via LfR receptor on intestinal enterocyte (Fe-LF transcytosis) without sulfate GI effects · NK + Tcell + sIgA immunomodulation · direct antimicrobial (pathogenic Fe sequestration + bacterial membrane permeabilization via lactoferricin peptide) · antiviral (HSV, COVID-19, rotavirus adhesion blockade). Stable up to pH 3 stomach in apo-form · partial gut digestion releases antimicrobial lactoferricin peptide.

«Bovine lactoferrin oral at 200-400 mg/day achieves hemoglobin restoration equivalent to ferrous sulfate with 67% fewer gastrointestinal side effects · this makes it the preferred iron repletion option for patients with intolerance to conventional iron salts.» Paesano · Italian Society of Hematology · BJN 2024

Apo-LF vs Holo-LF vs Liposomal · which to choose

Apo-LF (Fe-unsaturated) canon for anemia + antimicrobial · Liposomal boost oral bioavailability · Holo-LF specific niche.

Bovine apo-LF (Fe-unsaturated)<5% Fe · canon RCT form
ProsForm used in Paesano 2024 + Campione + most RCTs. Free Fe-binding capacity in gut (repletion) + antimicrobial (deprives pathogens). Stable at acidic pH.
ConsExpensive vs ferrous sulfate. Production requires EDTA desaturation + dialysis process.
Ideal use: anemia repletion · antimicrobial · antiviral · canon default.
Liposomal lactoferrinPhosphatidylcholine-encapsulated · absorption boost
ProsImproved oral bioavailability 2-3× vs standard apo-LF. Useful at lower effective doses. Some cardio/longevity studies use liposomal.
ConsExpensive (~€50-70/month). Few head-to-head studies vs standard apo.
Ideal use: longevity protocol · cardio · maximize absorption at lower dose.
Bovine holo-LF (Fe-saturated)≈100% Fe-saturated
ProsHigher direct Fe input (≈1.5 mg Fe/100 mg LF). Useful in severe Fe depletion cases when apo doesn't respond.
ConsLacks free intestinal Fe-binding capacity (main mechanism). NOT recommended first-line for anemia.
Ideal use: specific niche · NOT first-line. Apo-LF preferred as default.

Lactoferrin interactions · high safety profile

Excellent safety profile · specific clinical contexts: milk allergy · hemochromatosis · high dose pregnancy.

Consult healthcare professional if
 • Documented milk/casein allergy: bovine lactoferrin may contain casein/lactose traces (<1 ppm premium brands). Cross-reactions rare. Test under supervision first time if severe allergy. Functional allergology →
 • Hemochromatosis (Fe overload): CAUTION. Although apo-LF does not provide direct Fe, the enterocyte Fe absorption mechanism via LfR may increase Fe uptake. Under hematology if HFE+/overload.
 • High doses in pregnancy (>400 mg/d): data limited >400 mg/d during gestation. Paesano canon uses 100 mg × 2/d. DO NOT exceed without obstetrician.
 • Tetracycline / fluoroquinolone antibiotics: lactoferrin may chelate free Fe traces · space Fe-sensitive antibiotic 2-4h.
 • Children under 4 years: data limited <4 years additional supplementation to natural colostrum/breast milk. Under functional pediatrics.

Frequently asked questions about lactoferrin

8 real questions · answers based on Paesano + Campione + Bertuccioli + Zou literature.

Why is it better tolerated than ferrous sulfate?
Different absorption mechanism. Ferrous sulfate releases free Fe in gut → mucosal irritation + local ROS → nausea/constipation/dyspepsia. Lactoferrin: Fe absorbed via enterocyte LfR receptor in controlled transcytosis → 0 free Fe in lumen → 0 irritation. Paesano 2024 −67% GI effects.
Vegan? Vegetarian?
NOT vegan (bovine origin · milk). Yes vegetarian (no animal killing required). Some brands have human recombinant version produced in yeast/rice but VERY expensive and limited availability.
How long to notice effect?
Gestational anemia: Hb rises 30-60 days (Paesano). General anemia: ferritin 8-12 wk. COVID antiviral: 7-14 days treatment (Campione). Mucositis: during chemo/RT (Bertuccioli).
Apo-LF vs Holo-LF which?
Apo-LF (canon default). Free Fe-binding capacity + antimicrobial. Holo-LF already Fe-saturated → loses main mechanism. Canon RCTs use apo-LF.
Take on empty stomach?
YES. Improves absorption ~30-50% vs with meals. Empty stomach 30 min pre-breakfast or between meals. DO NOT take with dairy (competes absorption). Apo-LF stable at acidic stomach pH.
Is liposomal worth €50-70/month extra?
Depends on objective. Anemia: NO necessary · standard apo-LF sufficient Paesano canon. Longevity/cardio/maximize lower dose: YES (2-3× bioavailability boost). iHerb has Codeage Liposomal Lactoferrin.
Genetic hemochromatosis HFE+?
CAUTION. Although apo doesn't supply Fe, it improves enterocyte absorption via LfR. Under hematology mandatory if HFE+/Fe overload. Monitor ferritin every 4-8 wk if used.
Current COVID, does it work?
Campione 2024 RCT n=256 mild-moderate COVID: bLF 100 mg × 2/d × 10d · CT recovery −22%, IL-6 −31%. Mechanism: ACE2/spike blockade. Useful as adjuvant treatment for mild-moderate · does NOT replace paxlovid in high risk.
